Brattleboro – Care Coordination Core Skills and Care Conference Training

For new staff or those seeking a refresher in shared care.

The Care Coordination Core Skills and Care Conference training offers core skills for effective care coordination. Training topics include Eco Maps, Camden Cards, and Shared Care Plans.

Intended Audience: care coordination staff and leaders

Whether novice or expert, all levels find value in attending to advance your practice!

The Care Coordination Care Conferences training offers skill development for effective person-centered care conference facilitation.

Training topics include:

  • Person-centered care conference facilitation techniques with hands-on practice implementing shared care planning
  • Recommended format, location and considerations for various types of care conferences
  • Hands-on practice for 3 scenarios: people with stable medical or mental health conditions, crisis response, and goals of care/end of life

Up to 12 attendees, group learning format with some small group activities.

All trainings are offered regardless of participation with OneCare Vermont or Blueprint, and are offered free of charge.

All trainings are also open to attendees regardless of Health Service Area (e.g. trainings held in Brattleboro are intended to serve the communities of Windsor and Springfield, or other nearby communities). Seating is limited and preference will be given to registrants from the local health service area.
