• Hands-on practice for 3 scenarios: people with stable medical or mental health conditions, crisis response, and goals of care/end of life

Care Coordination training is offered regardless of participation with OneCare Vermont or Blueprint, and is free of charge. Care Coordination training is open to attendees regardless of Health Service Area or place of employment.
