Course Description:

Prepare and facilitate a patient- centered and strengths based care conference. Align goals and workflow, progress on goals in the shared care plan, assign team members for follow up care.

Learning Objectives:

• Set the right environment and tone for a patient-centered Care Team Conference
• Assign goals and tasks for the Care Team
• Identify achievements and barriers to compile goals in the shared care plan, including adverse Social Determinants of Health
• Summarize and identify next steps
