Course Description:

Provide information about the source information for the Social Risk Score in Care Navigator. Make suggestions about how to use the Risk score to prioritize outreach to patients.

• Provide information about the source data that comprises the Social Risk Score
• Understand how to use the Social Risk Score to prioritize outreach
• Share guidance about how to have sensitive conversations related to Social Determinants of Health
