OneCare is collecting performance data on the “Hypertension: Controlling High Blood Pressure” measure as part of the 2025 Population Health Model. By completing this online data collection attestation form, participants can earn compensation as set forth in the Population Health Model and Payments policy for 2025 (link at end of section below).

REPORTING PERIOD: March 1, 2024 to February 28, 2025.

REPORTING DEADLINE: To qualify for the YEAR-END Population Health Model payment, attributing participants must complete this online attestation no later than noon on Friday, March 14, 2025.
