Controlling High Blood Pressure

OneCare is collecting performance data on the Hypertension: Controlling High Blood Pressure measure as part of the 2024 Population Health Model. By completing this mandatory online attestation, participants have the ability to earn compensation as set forth in the 04-19-PY24 Population Health Model and Payments policy.

To qualify for the April Population Health Model payment, attributing participants must complete this mandatory online attestation no later than noon on March 15, 2024.
