OneCare is collecting performance data on mental health screening. By completing this online data collection attestation form, participants can earn compensation as outlined in the Mental Health Screening Incentive Program guidelines (2024 version).

REPORTING PERIOD: Reporting on the Mental Health Screening Initiative is designed to capture the rates of screening for mental health disorders for patient encounters from January 1, 2024 – December 31, 2024

REPORTING DEADLINE: To qualify for the January 2025 incentive payment, attributing participants must complete this online attestation of your mental health screening rates no later than noon on January 17, 2025.
