Data and analytics is an important component of the work OneCare is doing to transition health care in Vermont to a “value-based care” model. Providers who are part of OneCare can leverage OneCare’s data analytics to identify issues within a population and to rapidly develop targeted strategies to address those issues.

Pederson, MPH, COO of Brattleboro Memorial Hospital, describes how OneCare’s data was able to assist them in shaping their care approach in managing diabetic Vermonters who had poorly controlled A1C levels (a blood sugar measure). This inspired an unprecedented strategy in the approach to care for this specific population, leading to a dramatic improvement in health outcomes and a reduction in A1C levels.
