1. Performance Year 2024 Network Contracts Due August 15
On June 30, your ACO contract documents for next year’s participation were emailed to your hospital, practice, or organization’s contracting contact(s). Please return your contracts to contracting@onecarevt.org by August 15, 2023. Please plan accordingly. This year we have very little flexibility to extend the deadline.
2. October 2023 HSA Consultations
On July 25, we shared the agenda for the fall round of the OneCare health service area (HSA) consultations. This is a reminder to review the agenda and participants on the invitation and ensure your team is fully represented. The intended audience for this forum is executive-level contacts (CFO, COO, CMO, etc.).

Reminder–the purpose of these consultations is to discuss, create, and collaborate as a team so we can accelerate positive change in quality of care, stewardship of resources/cost containment, and coordination of the care delivered in each community.
3. 2023 UVM Health Network Beneficiary Notification Letter
OneCare will mail the 2023 UVMHN beneficiary notification letter to newly attributed individuals on August 4, 2023. For reference, a copy of the letter is available on the OneCare Portal, with the file name: 2023 Beneficiary Notification Letter – UVMHN Self Funded.pdf.
4. Care Coordination Reporting and Performance Rates
Care coordination trimester two reporting and narrative are due August 18, 2023. Please submit reporting via the OneCare Portal. The Population Health Model (PHM) quarter 2 HSA level reports with practice level performance rates are now available on the portal.
5. 2024 Population Health Model Measures and Training
OneCare hosted the first training for the 2024 PHM program on July 19; now available are the supporting PowerPoint slides and recording. Quality measure descriptions for the 2024 PHM measures are now available on the OneCare Vermont Portal.

The Q2 PHM performance reports by health service area, including practice-level insights, are now available on the OneCare Vermont Portal. These reports break down health service area performance by practice to provide greater insight into how individual practices compare within each HSA.

Reducing Readmissions through Improved Transitions of Care in the St. Albans Health Service Area
OneCare’s quality team has been working with Northwestern Medical Center to reduce their rate of hospital readmissions. Together, we created a plan to improve their “transition of care” to reduce readmissions—and therefore reduce health care spending and improve patient experience. Read the full story here.
