Colchester, VT – OneCare Vermont (OneCare), an Accountable Care Organization (ACO), dedicated to improving Vermont’s health care system, today announced key leadership transitions to lead the organization in its final year of operation.

Tom Borys, currently serving as OneCare’s chief financial officer, has been appointed interim CEO and CFO. He will replace Abe Berman, who was instrumental in founding OneCare in 2012 and returned to the organization as CEO in 2023. Berman will be stepping down in mid-December to join TapestryHealth, a health care organization combining human expertise and advanced AI-powered technology to deliver the highest level of innovative medical care for post-acute communities across the country.

Berman shared, “It has been a privilege to lead OneCare as we navigated significant changes in the health care landscape. I am grateful for the dedication of the team and the collaborative efforts of Vermont’s providers.”

“It is an honor to take on the role of interim CEO and CFO,” said Tom Borys, who has been a leader within the OneCare finance department since 2017. “I am proud of what OneCare has learned and accomplished over the last decade. Despite significant change around us, I see opportunity for continued progress and look forward to leading meaningful cost and quality improvement efforts through 2025.”

The OneCare Board of Managers also announced the election of Jennifer Gilwee, MD, to the position of board chair. Dr. Gilwee, who has served on the board since 2021, and currently chairs the Population Health Strategy Committee, brings a wealth of experience as a primary care physician and the division chief of general internal medicine at the University of Vermont Medical Center, and as an associate professor of medicine at the Larner School of Medicine at the University of Vermont.

“I am excited to step into the role of board chair and continue our work to enhance health care delivery across Vermont,” said Dr. Gilwee. “I look forward to continuing to support our providers as they deliver high-quality care to Vermonters.”

Dr. Gilwee will succeed Anya Rader Wallack, who has served as board chair since May of 2022. The board expressed deep gratitude for Wallack’s leadership and contributions during her tenure.

“Tom and Jennifer are both experienced leaders who will carry our mission forward as we work toward value-based care and high-quality health outcomes,” said Sunny Eappen, MD, MBA, president and CEO of UVM Health Network, OneCare’s sole member owner. “I also want to thank Abe and Anya for their commitment to the community and their leadership throughout a challenging reform environment,” Dr. Eappen said.

As OneCare continues its role in implementing Vermont’s All-Payer Model—a groundbreaking initiative aimed at improving care quality and controlling costs—Borys and Dr. Gilwee will oversee the organization’s work as it prepares to wind down operations by the end of 2025. The Board affirmed that all programs, investments, and waivers will continue through 2025 as planned and approved by OneCare’s Board of Managers.

Berman will remain in his role until mid-December to ensure a smooth leadership transition. Dr. Gilwee will officially begin her term as board chair in January 2025.
