Vermont Health Learn

Vermont Health Learn is a state wide e-Learn platform designed to support the health of all Vermonters through virtual courses for patients, providers, and community members.

Accessing Vermont Health Learn

Courses on Vermont Health Learn are created through a partnership between OneCare Vermont, the Department of Health Access, Blueprint, and the Vermont Department of Health. All of these organizations currently host over a dozen public facing courses for Vermonters.

You can register for and take these free courses anywhere internet access is available at any time. To learn more about accessing the courses you can read our Getting Started Guide for Learners.

Network Specific Resources

Vermont Health Learn houses network resources for our providers, care coordinators, and administrators. Below you can access the Quality Measures Course and the Care Coordination Corner, which includes recorded trainings.

 

This is your resource for relevant education and training modules including information, resources, and tools related to care coordination.

This course contains descriptive and detailed information relative to the ACO quality measures for each of our payer programs.
