Screening Application for Funded Waiver Projects

OneCare has allocated $100,000 to fund up to five delivery system projects intended to impact the key areas of quality focus and goals.
Minimum funding request: $10,000.

After a successful first round of funding, we’re proud to have supported several impactful projects. Now, we’re excited to announce a second round of funding for health care delivery projects that utilize waivers. Applications are due September 5, 2024 by 5pm. Please review the guidelines including the templates for the patient engagement waiver and participation waiver contracts, and find the button for application at bottom of this page.
